HealthQuest Pilot Study – Parent/Guardian Consent

Your child is invited to take part in the HealthQuest Pilot Study, a digital wellness program that teaches kids about healthy habits through videos, activities, and games. This pilot will help us learn how HealthQuest supports children and families in making positive changes.

What will happen:

  • Your child will use HealthQuest and complete short check-ins about what they learned.

  • You will complete brief surveys before and after to share how confident you feel supporting your child’s health.

  • We will collect anonymous data from app use and surveys to better understand how families engage with the program.

What we collect:

  • Anonymous responses from you and your child

  • In-app behavior (time spent, lessons completed)

  • General details like grade level or region

What are the risks or benefits?

  • No known risks. Your and your child’s names are never attached to your answers.

  • Possible benefits include improved family health habits, more confident parenting, and fun learning experiences for your child.

Your rights:

  • Participation is voluntary.

  • You may stop at any time.

  • Your decision will not affect your child’s care or services in any way.

You’re invited to try something fun called HealthQuest. It’s an app that helps kids like you learn about staying healthy—things like food, movement, and emotions.

What will I do?

  • Use the HealthQuest app to do quests and watch videos.

  • Answer a few short questions about how you feel and what you learned.

Is this safe?

Yes!
There are no wrong answers, and you don’t need to use your name. You can stop at any time if you don’t want to keep going.

Do I have to do it?

Nope! This is totally your choice. You can say “yes” or “no,” and you can change your mind anytime.

I want to try HealthQuest and share what I think. I understand that my answers won’t have my name, and I can stop anytime.
